RESUMO

INTRODUCTION: Attitudes and knowledge of health professionals are critical dimensions in the management of suicidal behavior. The "Suicide Behavioral Attitude Questionnaire" (SBAQ) was designed in Brazil to measure these variables in paramedical personnel. AIM: To translate and validate the SBAQ in literary Arabic. METHODS: After author-approved translation and back-translation, the Arabic version of the SBAQ was distributed to 125 Tunisian caregivers working in psychiatric departments. Factor analysis and internal consistency were calculated. The stability was measured overtime using the paired Student's T-test. RESULTS: Cronbach's alpha value was 0.741. The adequacy index of the factorization factor Kaiser-Meyer-Olkin (KMO) of our sample was 0.74. This value was higher than 0.5 which indicates that our data was suited for factor analysis. Bartlett's sphericity test result was significant, rejecting the null hypothesis. Three factors were extracted explaining 50.7% of the overall variance. Cronbach's alpha value for the three subscales was 0.735, 0.713 and 0.798. The instrument had good stability over time. CONCLUSIONS: The Arabic version of the SBAQ showed good reliability and validity allowing its use in Arabic speaking paramedical personnel.

RESUMO

Schizophrenia is associated to somatic disorders especially cardio-vascular and auto-immune. Through this case report, we describe an association with hypertrophic osteoarthropathy (HPO). For this patient, it was a paraneoplastic syndrome secondary to lung cancer. This syndrome is rare but important to recognize since it could hide a life-threatening condition.

RESUMO

INTRODUCTION: matricide is the murder of the mother. In this study, we report through a clinical case the circumstances of matricide and we underlined the predictive elements of the acting out. CLINICAL CASE: mister S, 31 years old had low socioeconomic conditions. He was a single man, unemployed and consumed alcohol. He was followed for a paranoid schizophrenia, but his therapeutic observance was bad, and he was aggressive with his mother. This aggressiveness was fostered by persecution and filiations' delusions. At the moment of the crime, his mother had appeared to him as so terrifying form that he had to kill her to defend itself. This clinical case confronts us with the difficulty of matricide prevention. If the short term prediction of the violence act was possible, the medium and long terms were harder. CONCLUSION: the regular evaluation of psychotic patients' dangerousness is needed to reduce the acting out risqué.

RESUMO

AIM: to study the prevalence of depression in type 2 diabetes and to seek for a relationship between glycemic control and treatment adherence. METHODS: This cross-sectional study was carried out at the outpatient department of the Tunis nutrition institute. A total of 100 diabetic patients followed for Type 2 diabetes were randomly recruited. We used a structured questionnaire to collect patients' socio-demographic, clinical and therapeutic data. The treatment adherence was assessed by using MAS, Glycemic control according to blood glucose and glycated hemoglobin (HbA1c) levels and depression and anxiety by administering a dialectal Arabic validated version of Hospital Anxiety and Depression Scale (HAD). Finaly we also used the modules on mood disorders of the mini international neuropsychiatric interview (MINI). RESULTS: The prevalence of depression in T2DP was 38% according to the HAD scale with severe forms in 18%. The anxiety prevalence was 31%. However, 18% of T2DP had anxious-depressive co morbidity. According to the MINI, 31% of T2DP had a mood disorder. Dysthymia was noted in 14% of cases, an isolated depressive episode in 6%. Depressed T2DP have a poor glycemic control than those no depressed. Macro-angiopathy, irregular followed-up, poor treatment adherence and unbalanced diet were associated with depression. CONCLUSION: The detection of depression related to somatic pathology in particular diabetes, seems essential. As a result, depression treatment may improve the overall prognosis of the associated somatic diseases.

RESUMO

The evolution of the legislation governing compulsory admission to hospital in Tunisia is interesting for two reasons. The country's 2011 revolution notably brought about major changes to the legislative framework, from constitutional through to ordinary laws. At the same time, the current trend for globalisation is also affecting legislation: international laws, treaties and UN charters are imposed on the laws of individual countries. This article looks at how Tunisian law governing compulsory admission to hospital has had to evolve.

RESUMO

Functional pain pathology is a public health issue, due to its prevalence, the costs it generates and the disability it causes the patient. The role of general practitioners is crucial. They must avoid the pitfall of pointless, sometimes dangerous, examinations and treatments, and seek to create with the patient a long-lasting and good quality relationship, key to the treatment of these disorders.
